Support Dog and Guide Dog Coats - Order Form

Ordering a custom-made coat from Blue Willow is easy. You can fill out the form here and pay via PayPal, print it out and mail it along with a check, or contact me and talk through your order on the phone.

All service dog coats are made-to-order and you will be sent a prototype coat for approval before the final coat is created. Please specify in the notes if you have special handling or shipping instructions. Don't hesitate to contact me if you have any questions!

Because of the additional design and labor effort involved in creating service dog coats, Blue Willow can only take service dog coat orders during our slower summer season: May 1st - August 15th of each year.
